Functional rhinoplasty refers to performing a nasal surgical procedure that involves modification of the external structures of the nose to improve its ability to permit easier passage of air flow. Structural issues that can compromise function can be congenital (innate to how the nose developed), trauma induced, or a consequence of aging. In this case, we straightened the nose in an effort not only to improve aesthetics but also breathing. When tackling such a challenge, it is helpful to determine what components—bony bridge, cartilaginous dorsal, nasal tip—are creating the crooked appearance. Upper lateral cartilages were released to free the dorsum from influences resisting change. The concave cartilaginous dorsum was “filled in” using a left spreader graft. A “clocking suture” pulled the cartilage dorsal complex to the left before fully reattaching the upper lateral cartilages. Making a deviated nose perfectly straight is an impossible task. Perfection lies in the hands of God. The process must be gradual to avoid a “blowout” of the earlobe, where the thin rim splits open due to excessive strain. But what happens when your big earlobe holes are no longer congruent with your lifestyle? Work situations change; social situations change. While those monster lobes may have looked cool when working at a tattoo parlor, they probably won’t cut it working at a bank. Removal of the over-sized jewelry will allow some shrinking of the earlobe. However, gauged earlobes over 10 mm in size are not likely going to return back to normal without plastic surgery. The patient shown used to wear 1-inch diameter gauge earrings, but he had them out for the last 5-6 years. The earlobe hole managed to shrunk to 3/4-inch diameter. The stretched lobe was repaired by freshening the edges and performing a double opposing closure: the lateral surface was closed horizontally, while the medial surface was closed vertically. This gentleman presented with long-standing nasal issues following an injury sustained years earlier while participating in mixed martial arts. Over time, the trauma led to a collapse of the nasal bridge, resulting not only in a visibly sunken profile appearance of the nose but also in significant nasal breathing difficulties. Restoring both form and function is fundamental to modern rhinoplasty. In this case, the goal was twofold: Rebuild structural support of the nose Improve airflow and breathing To accomplish this, we performed an external (open) rhinoplasty, which allows for precise visualization and reconstruction of the nasal framework. Auricular (ear) cartilage was used as a natural and reliable grafting material—providing essential “building blocks” to rebuild and augment the nasal dorsum.
